ALL Australians with type 1 diabetes are eligible for subsidised access to Continuous Glucose Monitoring (CGM) products, through the NDSS.

Sign up for a complimentary^# telehealth consult with a Credentialled Diabetes Educator (CDE) to assess your suitability for subsidised CGM products, such as FreeStyle Libre 2.

CDEs are authorised healthcare professionals who can submit CGM subsidy forms to the NDSS on your behalf.

You can use this once-off complimentary telehealth consult with a CDE to:

• Learn more about CGM products (including FreeStyle Libre 2), and the NDSS CGM subsidy
• Have your CGM subsidy application form signed and submitted to the NDSS, if you are not currently accessing any subsidised CGM products
• Have your CGM subsidy 'switch' form signed and submitted to the NDSS, if you are currently accessing subsidised CGM products and wish to change to a different CGM device (for example, if you wish to switch to FreeStyle Libre 2 from another device)

Upon completing and submitting this online form, you will be sent an email with your unique link to access your complimentary telehealth consult. You will have the option to speak to a CDE immediately^#, or to schedule a callback for a time that suits you.
